胃瘫是由胃排空障碍引起的，以恶心、呕吐、腹胀与腹痛为主要表现的临床综合征。这一系列慢性症状对患者的生理、心理及生活质量带来诸多不利影响。调查发现，胃瘫患者的死亡率远高于非胃瘫患者，这说明胃瘫早已对人类健康构成重大威胁，并逐渐成为一种严重的疾病负担。在过去的10年中，我们对胃瘫的病因与发病机制有了新的认识，也因此发现了一系列新的治疗措施。目前多认为胃瘫的发生与ICC细胞的减少引起的胃窦动力不足与节律失常有关。因此，我们对难治性胃瘫可采取治疗方式包括胃电刺激器植入术与幽门成形术。也有研究发现少数药物及手术治疗无效的胃瘫患者能从免疫疗法中获益。胃瘫患者常合并有焦虑或抑郁等心理障碍，心理评估与干预也应成为胃瘫治疗的重要组成部分。此外，寻找胃瘫的高危因素，及早采取预防胃瘫发生的措施，能更有效的防治胃瘫。最后，探索胃瘫治疗的新方法，如干细胞移植技术等，或许能为胃瘫的治疗带来新的希望。

 Gastroparesis (Gp) is a chronic disease that presents with clinical symptoms of nausea, vomiting, early satiety, bloating and abdominal pain, which is caused by delayed gastric emptying. These chronic symptoms have adverse impacts on the physiology, psychology and life quality of patients. Studies found that the mortality of patients with gastroparesis was higher than those without gastroparesis, which showed that gastroparesis has become a serious threa to human health. In last decade, there were some updates on the pathogenesis and management of gastroparesis. Studies found that gastroparesis were associated with decreased interstitial cells of Cajal (ICC), which caused the insufficience and allorhythmia of smooth muscle cells in the antrum. Therefore, endoscopic implantable gastric electric stimulator or pyloroplasty should be the choice for refractory gastroparesis. In addition, researchers found that some patients with gastroparesis who did not respond to drugs or surgery benefitted from immunotherapy. Not surprisingly, patients suffering from chronic gastrointestinal conditions frequently report psychological symptoms, such as anxiety and depression. So psychological evaluation and management should be considered for patients with gastroparesis. Moreover, identifying the risk factors of gastroparesis might help prevent occurrence of gastroparesis. At last, exploring novel treatment methods, such as stem cell transplantation, may bring new hope for gastroparesis.
