Application of “H2H” nutritional support combined with quantitative training in elderly gastrointestinal tumors

Application of “H2H” nutritional support combined with quantitative training in elderly gastrointestinal tumors

Abstract

Objective To explore the application effect of “hospital-to-home (H2H)” nutritional support combined with quantitative training on elderly patients with gastrointestinal tumors. Method A total of 152 elderly patients with gastrointestinal tumors who underwent surgical treatment in department of oncology of Beijing Shijitan Hospital, Capital Medical University were selected from April 2023 to February 2025. According to the random number table method, they were divided into intervention group and conventional group, with 76 cases in each group. Both groups were provided with routine perioperative nursing. The conventional group was given routine nutrition management and quantitative training, while the intervention group was given H2H nutritional support intervention on the basis of the conventional group. The postoperative recovery conditions (first exhaust time, first defecation time, first liquid intake time, hospitalization time) and nutritional status [patient-generated subjective global assessment (PG-SGA)], serum nutritional indicators [serum albumin (ALB), total protein (TP), hemoglobin (Hb)], physical activity indicators (number of daily walking steps, total daily walking distance) and cancer-related fatigue degree [Piper revised fatigue scale (PRFS)] before intervention and after 2 weeks and 4 weeks of intervention were compared between the two groups, and the complications were statistically analyzed. Result The first defecation time, first liquid intake time and hospitalization time in the intervention group were shorter than those in the conventional group (P<0.05). The PG-SGA score and PRFS score in the two groups were decreased after 2 and 4 weeks of intervention (P<0.05), and the scores in the intervention group were lower compared to the conventional group (P<0.05). The levels of ALB, TP and Hb in the two groups were increased after 2 and 4 weeks of intervention (P<0.05), and the levels were higher in the intervention group (P<0.05). The number of daily walking steps and total daily walking distance were decreased in both groups after 2 and 4 weeks of intervention (P<0.05), but the intervention group had higher indicators (P<0.05). The total incidence rate of complications in the intervention group was lower than that in the conventional group (2.63% vs 11.84%) (P<0.05). Conclusion “H2H” nutritional support combined with quantitative training can improve the nutritional status in elderly patients with gastrointestinal tumors, adjust serum nutritional indicators, slow down the decline of postoperative physical activity level, alleviate the cancer-related fatigue degree, and reduce the risk of complications.
