目的 探讨预后营养指数(PNI)指导下分级营养疗法对结直肠癌患者免疫功能、营养状况及并发症的影响。 方法 选取 2023 年 11 月至 2024 年 8 月于江苏省人民医院诊治的 155 例结直肠癌患者作为研究对象,按照随机数字表法分为观察 组(予以 PNI 指导下分级营养疗法,n = 78)和对照组(予以常规营养干预,n = 77)。 对比两组术后首次排气时间、进食时间、下 床时间和住院时间,观察入院时(T0 )、术前 1 d(T1 )、术后 7 d(T2 )、术后 30 d(T3 )的 PNI 和 T0 、T3 的血清 CD4 + T 细胞、CD4 + / CD8 + T 细胞、CD8 + T 细胞、白蛋白、血红蛋白、前白蛋白水平及并发症情况;采用 Spearman 法分析 PNI 与并发症的相关性。 结 果 观察组术后首次排气时间、术后首次进食时间、术后首次下床时间、住院时间均短于对照组,并发症发生率低于对照组(P< 0. 05)。 2 组术后 30 d 的血清白蛋白、血红蛋白、前白蛋白较入院时升高明显(P<0. 05);观察组术后 30 d 的血清白蛋白、血红 蛋白、前白蛋白均高于对照组(P<0. 05)。 观察组 T3 时点的 CD4 + T 细胞、CD4 + / CD8 + T 细胞、CD8 + T 细胞水平均高于对照组 (P<0. 05)。 观察组 T1 、T2 、T3 时点的 PNI 均高于对照组(P<0. 05)。 结论 PNI 指导下分级营养疗法在结直肠癌患者中有利于 改善营养状况和免疫功能,以及促进肠功能恢复与降低并发症发生率。

Objective To investigate the effects of graded nutrition therapy guided by prognostic nutrition index PNI on immune function nutritional status and complications in patients with colorectal cancer. Method A total of 155 patients with colorectal cancer who were diagnosed and treated in Jiangsu Provincial People 's Hospital from November 2023 to August 2024 were selected as the research subjects and divided into the observation group treated with graded nutritional therapy under the guidance of PNI n = 78 and the control group treated with conventional nutritional intervention n = 77 according to the random number table method. The time of the first postoperative exhaust the time of eating the time of getting out of bed and the hospital stay were compared between the two groups. The serum albumin hemoglobin and prealbumin of the two groups 30 days after the operation increased significantly compared with those at admission P < 0. 05 . The serum albumin hemoglobin and prealbumin in the observation group 30 days after the operation were all higher than those in the control group P<0. 05 . The CD4 + T cells CD4 + / CD8 + T cells and CD8 + T cells in the observation group at time point T3 were all higher than those in the control group P<0. 05 . The PNI of the observation group at time points T1 T2 and T3 was all higher than that of the control group P<0. 05 . Conclusion PNI guided fractional nutrition therapy is beneficial to improve nutritional status and immune function promote intestinal function recovery and reduce the incidence of complications in patients with colorectal cancer which is of great significance to improve the quality of life of patients.
