摘要

肌肉减少症(sarcopenia)是一种以进行性、全身性骨骼肌质量、力量和功能下降为特征的疾病,在临床上可分为原发性肌肉减少症和继发性肌肉减少症两种类型。肌肉减少症不仅显著增加老年患者衰弱、跌倒、失能和死亡的风险,也会导致外科手术患者及慢性病患者的临床预后不良。脂肪代谢紊乱在肌肉减少症骨骼肌丢失的发病机制中扮演着至关重要的角色。无论是原发性还是继发性肌肉减少症,脂肪代谢紊乱均可通过多种途径参与破坏骨骼肌稳态,包括异位脂肪沉积、脂毒性代谢物蓄积以及脂肪-肌肉内分泌轴失调等,最终导致蛋白质合成与分解失衡,进而可能加速肌肉丢失。因此,深入探讨脂肪代谢紊乱及其关键分子靶点对肌肉减少症的防治具有重要意义。本文结合最新研究进展与临床实践,系统分析脂肪代谢紊乱在肌肉减少症骨骼肌丢失中的作用及机制,旨在为肌肉减少症的精准防治提供新思路与理论依据,并强调在临床实践中重视脂肪代谢调控在防治肌肉减少症中的潜在价值。

Abstract

Sarcopenia is a progressive and systemic skeletal muscle disorder characterized by the loss of muscle mass, muscle strength, and physical function. Etiologically, it can be categorized into primary and secondary sarcopenia. Sarcopenia not only significantly increases the risks of frailty, falls, disability, and mortality among older adults, but also substantially worsens the clinical outcomes of surgical patients and those with chronic diseases. Emerging evidence underscores a crucial role for dysregulated lipid metabolism in the pathogenesis of sarcopenia-related skeletal muscle loss. In both primary and secondary sarcopenia, dysregulated lipid metabolism contributes to the disruption of skeletal muscle homeostasis through multiple mechanisms, including ectopic fat deposition, accumulation of lipotoxic metabolites, and dysregulation of the fat-muscle endocrine axis. Collectively, these alterations impair the balance between protein synthesis and degradation, thereby possibly accelerating muscle wasting. Consequently, elucidating the molecular mechanisms underlying dysregulated lipid metabolism and identifying new therapeutic targets are of great significance for the prevention and treatment of sarcopenia. Integrating recent advances in research and clinical practice, this review comprehensively and systematically summarizes the roles and mechanisms of dysregulated lipid metabolism in sarcopenia-related skeletal muscle loss, aiming to provide novel insights and a theoretical framework for the precision management of sarcopenia, and to highlight the therapeutic potential of targeting lipid metabolic pathways.
